I rarely recommend any programs such as this however I have been taking these
surveys for years and turn in my points at the end of each year for

What you need to know: These open in adobe reader 7 or above so you can add your own text at the top. Then Print the page > Cut Out > Glue or tape to Jewel Case cover. Attach to CD or DVD Jewel case or soft fold cover. Not recommended for attaching directly to CD or DVD.

Ideas: Send your student camcorder movies, CD of their favor songs, family message or use the camcorder to go take the sporting games of their favorite high school sport team so they can keep up with friends. You can even invite their friend over to record a camcorder message (or take pictures using a camera) to let all their friends at home send them messages. Yes, all this can be done by emails today but a care package box from home with CD and/or DVD with some cookies and candy is a lot more fun!

For a pdf free printable: Most computers come with an Adobe Reader free
software and/or software in your web browser that will open pdf
